When you visit Chattahoochee Family Orthodontics (“CFO”), you don’t have to deal with the hassle of submitting insurance claims. We will submit all claims directly to your insurance company and work with the insurance provider to make sure your coverage meets your needs and budget. We will provide the cost of your orthodontic treatment and discuss available payment options with you before you begin, so you can make the best choice for you.

To enable us to have up-to-date and accurate billing information in our system, we kindly request that all patients bring the following to every visit:

  • Current insurance card(s)

  • Photo ID

  • Money Orders

  • Visa, MasterCard, American Express, Discover

Your Treatment Plan—Making Your Payments

The CFO Treatment Coordinator discusses payment options with you when you are making the decision to start treatment. Payment in full and/or payment plans are available and will be discussed at that time. The TC will obtain the amount of insurance your carrier will allow and factor that into the cost of your treatment plan. Your insurance company will determine benefit reimbursement and any remaining balance is your responsibility.

Payment for Services

All payments are expected to be paid according to the treatment plan schedule. We are often able to work out special payment plans for patients with extenuating circumstances. For further assistance regarding insurance or billing please contact our office at 770-415-8439.

Check Policy

All checks must be made out to Chattahoochee Family Orthodontics. A service charge of $30.00 will be assessed for each returned check.

Orthodontics Insurance & Claims Submission

As a service to our patients, we will provide an estimate of costs prior to services rendered. Chattahoochee Family Orthodontics will submit all claims directly to the patient’s insurance company. The insurance company will determine the benefit reimbursement. Any remaining balance is the patient’s responsibility.

Additionally, please be aware that not all insurance carriers offer orthodontic coverage. Chattahoochee Family Orthodontics accepts certain providers offering this coverage; however, if your provider is not on our list, CFO will be considered “out-of-network” and as previously discussed, we will process the insurance claims on the patient’s behalf.

Coverage Changes

Orthodontic insurance pays our office in installments over the length of your treatment, if your insurance coverage ends or changes please inform us immediately.  Providing us with this information prior to your visit will reduce your wait time. We will also do our best to verify your insurance coverage prior to your first consult visit to minimize surprises at the time of visit.

Non-Payment

Our practice is committed to providing the best possible treatment for our patients at a fair and reasonable cost. Payment is expected according to the treatment plan contract. If your account is more than 90 days past due, your account may be turned over to an outside collection agency. These agencies charge fees. You agree to be liable for all such collection expenses, legal fees, and court costs. If you are dismissed from the practice as a result of non-payment, you will be notified by certified mail that you have 30 days to find alternative medical care. During that 30-day period, our providers will only be able to treat you for emergency care.

Dental Insurance We Accept

When you visit Chattahoochee Family Orthodontics (“CFO”), you don’t have to deal with the hassle of submitting insurance claims. We will submit all claims directly to your insurance company. As a service to our patients, we will provide an estimate of costs prior to services rendered. Your insurance company will determine benefit reimbursement and any remaining balance is your responsibility.

CFO accepts all PPO insurance plans and is in-network with Aetna.

For all other carriers, Chattahoochee Family Orthodontics is “out-of-network”; however, as a courtesy, we will process all insurance claims on the patient’s behalf.
